High Risk Foot Services in regional Victoria

Includes those that have:

Capacity to manage the most complex, high risk foot
24 hour access to an operational Emergency Department
Minimum of one medical specialist on the inter disciplinary team
Please note that other services may exist that can manage the high risk foot of lower complexity

	Gippsland		Latrobe Community Health Service High Risk Foot Clinic		Individuals with complex foot conditions placing them at high risk of hospitalisation and amputation such as foot ulcerations, infections and suspected Charcot neuroarthropathy Phone: 1800 242 696 Email: MHRFClinic@lchs.com.au
	Grampians		Ballarat Health Services High Risk Foot Clinic		People with complex high risk foot issues and complications, including: foot ulcerations which are not progressing, history of amputation and current foot wound, suspected Charcot neuroarthropathy, complex foot pain.
	Hume		East Subregion Gateway Health High Risk Foot Service, Wangaratta		People with foot pathologies placing them at a high risk of amputation (eg. Foot ulceration, infection, Charcot’s Neuroarthropathy) related to conditions including, but not limited to: Diabetes, peripheral arterial disease, rheumatoid arthritis, neurological impairment or immune disease.
	Hume		West Subregion Goulburn Valley High Risk Foot Clinic		People with active foot pathologies that place them at risk of hospitalisation and require multidisciplinary assessment, such as active foot ulceration, recent amputation, infection and suspected Charcot’s Neuroarthropathy.
